PPO
Plan Coverage Period
  • Minimum: 10 days
  • Maximum: 2 years
  • Benefit Period: 6 month
Plan Maximum
  • Under 65 years of age: $50,000, $100,000, $500,000, $1,000,000 per each injury or sickness.
  • For ages 70 - 79: only $50,000 of coverage is available.
  • US$2,000,000(life of the policy) option is not for each injury.
Deductibles
  • $0, $100, $250, $500, $1000, $2500 for every injury or illness
Pre-existing Conditions Coverage
For US Citizens Only:
Sudden and Unexpected Recurrence of a Pre-existing Condition (Medical):
Up to age 65 with primary health plan - usual, reasonable and customary charges up to plan maximum;
Up to age 65 without primary health plan - $20,000 lifetime maximum ;
65 years and above with or without primary health plan - $2,500 lifetime maximum
Coverage Benefits
  • Emergency Medical Evacuation:Up to $500,000
  • Emergency Reunion:Up to $50,000
  • Return of Mortal Remains:Up to $50,000 of Return of Mortal Remains
Eligibility
  • US Green card holders.
  • If the Immigrant is above 65 years of age: Plan must be purchased within 30 days
Co-insurance: Within US
  • Treatment received within the PPO Network: Patriot GoTravel will pay 90% of the expenses up to $5,000, then 100% up to the Maximum Limit
  • Treatment received out of the PPO Network: Patriot GoTravel will pay 80% of the expenses up to $5,000, then 100% up to the Maximum Limit
Renewal
  • If the plan has been bought for a minimum of 1 months, it can be renewed for up to 2 years without any break in the coverage.
  • Online Renewal is available in whole month or daily increments.
  • For each renewal of 1 month done online, an additional processing fee of $5 will be charged.
Optional Coverage
  • Adventure Sports Rider: Available to insured up to 65 years. $50,000 (0-49 years); $30,000 (50-59 years); $15,000 (60-64 years)
  • Enhanced AD & D Rider: Available to primary insured only. Up to an additional $400,000
  • Citizenship Return Rider: Up to maximum limit
