Pre-existing Conditions Coverage
For US Citizens
Sudden and Unexpected Recurrence of a Pre-existing Condition (Medical):
 Up to age 65 with primary health plan - usual, reasonable and customary charges up to plan maximum;
Up to age 65 without primary health plan - $20,000 lifetime maximum ;
65 years and above with or without primary health plan - $2,500 lifetime maximum
Sudden and Unexpected Recurrence of a Pre-existing Condition (Emergency Medical Evacuation):
Up to $25,000 of eligible costs and expenses
For Non US Citizens Sudden and Unexpected Recurrence of a Pre-existing Condition (Medical):
 Up to age 65 - $50,000 lifetime maximum for eligible expenses
Age 65 years and above - $2,500 lifetime maximum
Sudden and Unexpected Recurrence of a Pre-existing Condition (Emergency Medical Evacuation):
Up to $25,000.
Precertification
  • The insured or the doctor treating the insured must contact the number mentioned in the IMG ID Card before admitting the insured to the hospital or performing a surgery.
  • The billing will be done instantly if precertification is obtained.
Eligibility
  • US Green card holders.
  • If the Immigrant is under 65 years of age: Plan must be purchased within 6 months of arrival to US
  • If the Immigrant is above 65 years of age: Plan must be purchased within 30 days
Co-insurance: Within US
  • Treatment received within the PPO Network: No Coinsurance
  • Treatment received out of the PPO Network: Patriot Platinum will pay 90% of the expenses up to $5,000, then 100% up to the Maximum Limit
Renewal
  • If purchased for a minimum of one month, Patriot Platinum may be renewed for a total of up to three years.
  • It can be renewed in whole month or daily basis online or by paper applications. However renewals for less than a month can only be done online
  • For renewal of policy for less than a month, additional $5 processing fee will be charged.
Optional Coverage
  • Maximum coverage for Adventure Sports is up to $50,000
  • Maximum Coverage for Enhanced AD&D is up to $400,000 only for the primary insured
  • Maximum Coverage for Citizenship Return up to max plan limit
  • Evacuation Plus Rider available up to the age of 65 years
  • Personal Liability
    • Injury to third party: $2,000 limit after $100 deductible
    • Injury to third person property party: $500 limit after $100 deductible
Additional Benefits
  • Return of Minor Child : Up to $100,000 if the insured person is hospitalized as in-patient, or dies
  • Return of Mortal Remains : Up to $50,000 for Return of Mortal Remains or $5,000 for Cremation/Burial
