Plan Details

PPO
Eligibility
  • Green Card holders or new immigrants
  • For the age group of 15 days to 74 years
  • Applicants must be outside USA at the time of application, or plan to depart USA within 30 days of the Certificate’s Effective Date
Coverage Period
  • Minimum: 1 year
  • Maximum: Lifetime(up to 75 years of age)
  • Coverage valid in US only for 6 months out of the 12 period
Deductibles
  • $70, $100, $150, $250, $500, $1000 per person per injury/sickness
Lifetime Maximum
  • Up to $500,000
Coverage for Children
  • If both parents are covered under Reside, the first 2 dependent children between 14 days through 9 years of age will also be covered free of charge
Newborn Child Coverage
  • Restricted to first 31 days of life up to $5000
Refund Process
  • The total plan cost will be refunded if a written request is received by Seven Corners prior to the Effective Date of Coverage.
  • If written request is received after the Effective Date of coverage, the unused portion of the plan cost may be refunded minus a cancellation fee, provided no claim has been submitted to Seven Corners for reimbursement.
